Joining up health and social care
One of the biggest challenges facing the care system in Scotland is how to bridge the gap between health and social care services. Making sure that health and social care services talk to each other is important so that people get the best care and support to meet their needs.
At the beginning of 2014 we made a step forward with our campaigning on this issue. The Scottish Parliament is now trying to get this right by integrating health and care services – placing people and communities at the heart of everything so that people get the right care in the right place at the right time.
In particular, the new laws hope to address the problem of ‘bed blocking’ - where people have to stay in hospital because of a lack of social care in their community - and to support people to live as independently as possible for as long as possible.
